Syncope in patients who have known structural heart disease or abnormal ECG is most often caused by arrhythmias.
The following investigations should be considered:
- echocardiography in most patients
- exercise testing is particularly informative if syncope occurs on exertion
- 24 hour ECG monitoring
A referral to a cardiologist should also be considered.
