paroxysmal myoglobinuria

  • a necrotizing disease affecting exercising muscles

  • features include:
    • muscle tenderness, weakness, swelling and bruising
    • abdominal pain, fever, pallor, disseminated intravascular coagulation and shock
    • urine discolouration - initially urine turns pink then, as a result of increased myoglobin excretion, turns red/brown
    • acute renal failure may occur
    • laboratory features:
      • raised white cell count, raised liver function tests, features of DIC, hypoxia
      • diagnosis via muscle biopsy; also raised CK and serum myoglobin
