paroxysmal myoglobinuria

- a necrotizing disease affecting exercising muscles
- features include:
- muscle tenderness, weakness, swelling and bruising
- abdominal pain, fever, pallor, disseminated intravascular coagulation and shock
- urine discolouration - initially urine turns pink then, as a result of increased myoglobin excretion, turns red/brown
- acute renal failure may occur
- laboratory features:
- raised white cell count, raised liver function tests, features of DIC, hypoxia
- diagnosis via muscle biopsy; also raised CK and serum myoglobin
