evaluation/staging of pelvic organ prolapse

Two main systems are used for staging the degree of pelvic organ prolapse:

  • Baden-Walker system

    • grade 0 - normal position for each respective site, no prolapse
    • grade 1 - descent halfway to the hymen
    • grade 2 - descent to the hymen
    • grade 3 - descent halfway past the hymen
    • grade 4 - maximal possible descent for each site

  • pelvic organ prolapse–quantification

    • stage 0 – no prolapse
    • stage I - > 1 cm above the hymen
    • stage 2 - ≤ 1 cm proximal or distal to the plane of the hymen
    • stage 3 - > 1 cm below the plane of the hymen, but protrudes no farther than 2 cm less than the total vaginal length
    • stage 4 - eversion of the lower genital tract is complete
