new patient registration

A new patient registration consultation must be offered to all patients over 5 years old. The consultation is to establish relevant past medical and family history including:
- medical factors - illnesses, immunizaitions, allergies, hereditary factors, screening tests, current health
- social factors - employment, housing, family circumstances
- lifestyle factors - diet and exercise, smoking, alcohol and drug abuse
The examination must include height, weight and blood pressure.
