In paediatric fluid balance, unless there is shock, an attempt would be made to replace the defecit over the first 24 to 36 hours. The deficit is calculated on clinical grounds - ie., 5%, 10% or more dehydrated. Thus a 30 kg child who is estimated to be 10% dehydrated requires 3 kg replacement - ie. 3 litres.
In shock one would use:
- 10 to 20 ml/kg 4.5% albumin or normal saline, followed by
- rehydration over 48 hours
This may alter between centres and clinical situations.
