There is good evidence that patients with symptomatic carotid artery stenosis >70% benefit from carotid endarterectomy.
In calculating whether surgery is indicated for a particular patient one should consider:
- the risk of further strokes:
- TIA's suggest high risk
- large completed strokes reduce the utility of ipsilateral surgery
- the operative risk of complications:
- operator dependent
- avoid surgery soon after a stroke or myocardial infarct
