The most important prognostic factor for renal adenocarcinoma is the extent of tumour invasion at the time of diagnosis:

  5 year survival
confined to kidney 75%
invading perirenal fat 50%
involving regional lymph nodes or renal vein or IVC 35%
distant metastases 10%

About half of those who have curative resection for earlier stages of the disease also go on to develop advanced and/or metastatic disease (1).
