ACE inhibitors interact with a number of drugs:

  • diuretics:
    • patients previously administered diuretics on a long-term basis have a high incidence of first-dose hypotension; this is minimised by reducing the diuretic dose for at least 24 hours before ACE inhibitors are instigated
  • lithium - ACE inhibitors raise the plasma concentration
  • antipsychotic drugs - ACE inhibitors precipitate postural hypotension
  • NSAID's reduce the antihypertensive effect of ACE inhibitors
  • cyclosporin - increases the risk of hyperkalaemia
