comparison of Alzheimer's disease and DLB

The following features may help to differentiate Alzheimer's disease (AD) from dementia with Lewy bodies (DLB):

  • memory loss:
    • AD: usually the presenting feature with significantly worse memory functioning
    • DLB: memory may be relatively normal in the early stages
  • course:
    • AD: progressive
    • DLB: progressive with marked fluctuations
  • parkinsonism (unsteady gait, bradykinesia, cogwheeling):
    • AD: late stages
    • DLB: early stages
  • hallucinations:
    • AD: not a major feature
    • DLB: a hallmark feature.
