perinephric abscess

This may occur as a complication of pyelonephritis, particularly if there is a large 'staghorn' calculus present, when the pus discharges through the renal capsule into the perinephric fat. Also may develop as a result of haematogenous infection of a traumatic perinephric haematoma.
Presentation:
- systemic symptoms of infection
- local low-grade symptoms
- pyuria
Investigations:
- ultrasound or radiology may reveal a non-functioning renal mass which contains a fluid level
Treatment:
- drainage of the abscess
- nephrectomy may be required
