Lisinopril is the only ACE inhibitor passively excreted by the kidney - it should be safer than other ACE inhibitors in patients with dubious renal function. Other ACE inhibitors are excreted to a variable degree by active tubular excretion.
Also, lisinopril is not a prodrug, so it doesn't need liver metabolism. At 10mg daily gives 24hr BP control whereas with Enalapril need 20mg to give 24hour control.
Start 2.5mg od for seven days, which usually has no therapeutic response. The normal dosage is 10 to 20mg daily for hypertension, 5 to 20mg as an adjunct therapy for heart failure. The maximum does is 40mg, less in the elderly.
