Having palpated the iliac tubercles and with the thumbs of both hands still positioned on the anterior superior iliac spines, the fingers are advanced inferiorly. After about 8-10cm, bony prominences are palpated bilaterally - the greater trochanters of the hips. The posterior edge is less shielded by muscle and easier to palpate.

The examiner must determine if the two trochanters are at the same level. Fracture or dislocation of the hip may result in different horizontal levels.
