The presence of bacteria on urine microscopy provides immediate evidence of urinary tract infection. Gram staining may help identify the pathogen.

If the bacteriuria is accompanied by pyuria, urinary tract infection is more likely. However, a mid-stream specimen must also be sent for quantitative culture and sensitivities before therapy is initiated.

A poorly collected urine specimen may well contain urethral and perineal organisms.
