Parathyroid gland deficiency:

  • primary deficiency:
    • idiopathic / congenital pseudohypoparathyroidism
    • DiGeorge syndrome

  • secondary deficiency:
    • destruction during thyroid, parathyroid or laryngeal surgery; by far the most common cause
    • infiltration by iron in haemochromatosis or by metastases

Reduced secretory activity:

  • hypomagnesaemia - Mg necessary for PTH secretion
  • vitamin D excess

End organ resistance:

  • pseudohypoparathyroidism
  • hypomagnesaemia
