These include:

  • developmental feature: puberty
  • familial feature
  • insufficient sleep
  • malnutrition
  • psychological factors e.g. school phobia, depression
  • hypoglycaemia
  • anaemia
  • infection - low grade, such as pyelonephritis, tuberculosis, persistent haemolytic streptococcal throat infection, or early rheumatic fever
  • apical tooth infection
  • glandular fever
  • chronic or subclinical hepatitis
