Submucous or subendometrial fibroids arise in the submucosal layer of the uterus. They usually originate from intramural tumours which grow towards the endometrial cavity.
They may become pedunculated, project through the cervix, and present as a leiomyomatous polyp. By this stage, the polyp of the stalk is usually quite elongated so that the tumour is at risk of ischaemic necrosis and subsequent infection.
Submucosal fibroids often present with colicky uterine pain as the uterus contracts in an attempt to expel the tumour.
