The mesangial localisation of the IgA complexes suggests entrapment of circulating immune complexes or aggregates. 50% of patients with Berger's disease have elevated IgA levels.
The nephropathy occurs by activation of the alternate complement pathway.
There appears to be an inherited defect in the handling of IgA in these patients.
There is an association with HLA B35 and D4.
