The most useful film is the erect chest radiograph, in which can be seen air under the diaphragm. This, however, has an important differential diagnosis as follows:

  • perforation from the gut
  • from outside, for example in trauma, surgery or peritoneal dialysis
  • from the lung, through the diaphragm
  • as a result of a gas infection
  • through the vagina, for example after waterskiing
