During a hypercyanotic attack the infant becomes restless and hyperpnoeic as cyanosis increases and gasps for breath. Syncope may occur.
In severe attacks there may be loss of conciousness and convulsions.
On examination there is a reduction in the loudness of the murmur. If a jugular venous pressure can be seen there is no a wave.
