Causes include:
- hypothyrodism
- malnutrition
- nephrotic syndrome
- Cushing's syndrome / patients on corticosteroid therapy
- acromegaly
- patients with congenital absence of thyroid-binding globulin
- drug therapy:
- mercurial diuretics
- androgens and anabolic steroids
- diphenylhydantoin therapy
