cyclosporin and St John's wort

St John's wort interacts with cyclosporin and leads to reduced cyclosporin levels.
Suggested management of patients already taking SJW preparations:
- check cyclosporin levels and stop SJW
- note that there may be an increase in cyclosporin levels on stopping SJW
- it may be necessary to adjust the cyclosporin dose
