Special care should be taken because a severe Jarisch-Herxheimer reaction may occur. This causes a fever, tachycardia and may weaken the aortic wall. If the coronary ostia are involved myocardial infarction may occur.
To reduce the severity of this reaction the patient should be placed on prednisolone.
