The patient is shocked.

There may be haemorrhage and the uterus may appear at the introitus.

Shock is more severe than expected from the degree of blood loss.

Unexplained shock in the third stage of labour is an absolute indication for a vaginal examination to rule out acute inversion of the uterus. The uterus will be felt protruding from the cervix.

Untreated inversion of the uterus has a high mortality.
