The clinical features of spondylolisthesis varies according to the underlying cause.

Inflammatory causes in children are most often due to an acute infective lesion in the ear, throat or neck; those in adults, to rheumatoid arthritis. The patient complains of a stiff neck. The head is held rigidly with the cervical muscles in spasm.

Patients with congenital deficiencies of the dens or instability of the cervical vertebrae present with discomfort and stiffness in the neck. A flexion deformity is apparent.
