Keller's operation

Keller's excision arthroplasty is performed to correct severe hallux valgus deformity.
It involves:
- the excision of the bunion (exostosis) on the first metatarsal
- the excision of the proximal part of the first proximal phalanx
- (occasionally) lengthening the tendon of extensor hallucis longus
The great toe is left shorter and more floppy than normal. A disadvantage to the procedure is that shortening of the toe allows the sesamoid bones to slip backwards which may cause pain in the forefoot.
The operation is suited to older patients with arthritic joints and secondary deformity.
